Help Creating Own Facepack

How do you find out a players code/ID to create a face picture for them? I downloaded the Facepack from here but it doesn't have all of my players in (I'm playing as Salford) so I've created some of my own. But I don't know what I need to call the files to place into the folder.

Here iS the list for Salford club, UID and Name.

IF need more , you can use search tools in sortitoutsi.net To find player UID if who iS in FM DB.
